How do I set an appointment with you?
Contact Dr Miriam Lee to confirm a day and time that best suits you to see Dr Lee. In order to obtain a rebate from Medicare you will need to have a referral.
You can obtain a referral from your GP, who may give you a 12 month or indefinite referral. A referral from another specialist will last for only 3 months from the date of your first consultation.
If you are seeking medical advice as a couple, it is a Medicare requirement that the names and dates of birth of both partners appear on referrals.
What do I need to bring to my appointment?
Please bring your referral (even if this has already been sent to the Doctor), your Medicare card and any Private Health Insurance cards or details.
If you have begun any infertility investigations with your GP or another specialist, please bring along any test results or paperwork.
